Preparing for your first appointment at Retina of North Texas

A patient’s initial appointment may take 2-3 hours and includes a complete eye examination with dilation. During the exam, we will use eye drops to dilate your eyes, causing temporary blurred vision, so it may be necessary for someone to drive you home after your appointment. For your convenience, we perform significant ocular diagnostic testing during these eye appointments and the doctor will review the results of these tests during your visit to avoid a follow-up visit to review testing results. Decisions on treatments based on diagnositc testing will often be made during the visit, and a procedure to treat your retinal condition may be offered on this initial visit. It is wise to bring a chaperone/driver to the initial visit in the event the patient experiences blurry vision after the procedure.


If you were referred to Retina of North Texas by another eyecare provider or physician, please bring a copy of your medical and eyecare records or at least your last clinic note or patient summary which can often be downloaded through a patient portal. Sometimes medical records sent to us don't always make it to us in a timely fashion, so it's worthwhile to bring a hard copy with you.


Be sure to let the receptionist know if you will require any special help for your visit, such as a wheelchair, and please bring a driver if it's your first visit and you may require an injection of medication.


For your convenience, you can download our new patient forms below prior to your visit , complete them, print them out and fax or e-mail them to us. Please tell the receptionist whether you plan to fill in and fax these forms to us. Do not worry if you’re not able to fill in every question. Our staff will work with you to fill in any gaps in your paperwork.



If you are uncertain about the details of your insurance coverage, our office staff will work with you to help determine and understand your coverage, including any preauthorizations required before surgical procedures. Please bring a physcial copy of your insurance card with you on all visits, and let us know if there has been a change in your insurance coverage.
